Type 1 vs. Type 2: Why Your Diabetes Type Determines Your Emergency Survival Strategy

1. The Supply Chain (Building the Buffer)

  • “To protect myself against supply chain disruptions, I want to build a 90-day buffer of my daily medications. Can you write my prescription for a 90-day supply rather than 30?”
  • “If insurance won’t cover a backup supply, can you write a separate prescription for generic alternatives (like Metformin or older insulin types) that I can pay cash for to keep as an emergency reserve?”
  • “If I run out of my specific medication, is there a ‘close enough’ alternative likely to be found in a standard first aid kit or another family member’s medicine cabinet that I could use in a life-or-death situation? Or is that strictly dangerous?”

2. The Cold Chain (Insulin Users Only)

  • “If the power fails and I lose refrigeration, exactly how long will my specific brand of insulin remain effective at room temperature (approx 75°F)? Is it strictly 28 days, or is there a margin of error?”
  • “In a worst-case scenario where I cannot keep modern analog insulin cool, should I have a backup supply of NPH or Regular insulin (which are often more temperature stable)? If so, what would the conversion dosage look like for me?”

3. The Survival Diet & Activity

  • “Most emergency food storage is high-carb (rice, beans, pasta). If my diet shifts suddenly to 70% carbohydrates, how should I adjust my insulin-to-carb ratio or my oral medication dosage?”
  • “In a disaster, I will likely be doing heavy physical labor (hauling water, chopping wood). How drastically does intense physical exertion drop my blood sugar, and how much less insulin should I take on those days to prevent going low?”

4. The “No 911” Rescue (Hypo/Hyperglycemia)

  • “If I experience severe hypoglycemia (low blood sugar) and an ambulance cannot come, should I have a Glucagon emergency kit (injection or nasal powder) on hand? Can you prescribe one and teach my spouse how to use it?”
  • What is the shelf life of a Nasal Powder (Baqsimi), Auto-Injectors (The “EpiPen” Style) like Gvoke HypoPen / Gvoke PFS, and Traditional Injection Kits (Powder + Water)like GlucaGen HypoKit (Novo Nordisk) or Generic Glucagon Emergency Kit (Amphastar/Lilly)
  • Does the expiration date start when I pick up the prescription?
  • Is there a way to extend the shelf life of the traditional injection kits?
  • Which Glucagon option handles heat the best?
  • “If I experience hyperglycemia (high blood sugar) and cannot go to the ER, at what glucose level does it become immediately life-threatening? Do I need ketone test strips at home?”
  • “If I have ketones present but am alert, what is the protocol for ‘sick day’ management to flush them out without IV fluids?”

5. Wound Care & Infection

  • “Since diabetics heal slower, what specific antibiotic ointment or wound care supplies should I have in my kit to treat minor cuts on my feet before they become infected?”
  • “If I get a foot infection and cannot see a doctor, are there any oral antibiotics you would feel comfortable prescribing me in advance to keep in my emergency kit for that specific risk?”

Important Note for the Patient

When you get these answers, write them down immediately. In a high-stress emergency, you will not remember the specific dosage conversion or the shelf-life days your doctor told you. Put the written answers in a Ziploc bag with your backup medication.

Get their answer. Write it down on pen and paper. That is your plan. This article is just the wake-up call to go get it.
